PASA Waiver Services

Foothills Behavioral and Education Consultants is a program approved service agency (PASA).  We provide direct services through several of the Home and Community-Based waivers to individuals with intellectual or developmental disabilities approved for Medicaid waiver services. 

Adult Behavioral Services

Overview of Home and Community Based Adult Behavioral Services: Tailored for adults, behavioral services promote independence, improve social interactions, and support the development of life and vocational skills that facilitate participation in the community. 

Counseling Services 

Individual/Group Counseling Services are designed to address identified needs and challenges. Counseling can be provided in individual or group settings depending on the needs of the individual.  We provide evidence-based and tailored interventions to meet the needs of each individual served. 

Evidence-based counseling techniques such as Cognitive Behavior Therapy, Systematic Desensitization, Anger Management, Biofeedback, Relaxation Therapy and other empirical strategies may be used to help individuals develop appropriate adaptive behaviors, improve interactions with others, and manage their behavior more effectively.

Location of counseling services can vary depending on the preferences and needs of the individual. For example, counseling sessions may be conducted in the home, the community, a day program setting or another participant preferred location. 

In-home counseling: For individuals who have difficulty traveling or who prefer to receive services in the comfort of their own home.

Community-based counseling: Counseling services received in the community setting may be familiar and comfortable, such as in a local community center, large or small activity room in a day program, at a public library or another preferred community setting.

Clinical counseling: For individuals who prefer a more formal counseling setting, counseling services may be provided in a day program or community center board office.  This setting may also be a good option for individuals who require a more structured environment for counseling sessions.

Consultation Services 

Behavioral Consultation Services for eligible individuals on the DD and SLS waivers may require assistance in developing appropriate behaviors and interacting with others. These behavioral services involve consultations and recommendations for behavioral interventions and the development of behavioral support plans. 

The location where behavioral consultation services are provided vary depending on individual's needs and preferences, such as the individual's home, a community setting, a day program or another location as determined by their service plan.

Assessment Services 

Assessment is the cornerstone of all person-centered treatment plan development.  Behavioral Plan Assessment services are individualized for each program participant we serve. Typically, a functional behavioral assessment includes direct observations of the individual in various settings for the clinician to collect information about pertinent skills and behavior. 

Assessment interviews are conducted with those who interact with the individual the most (often direct care staff and caregivers) about their behavior. The individual's preferences and personal goals are embedded throughout the process and to the greatest extent possible they're included in all aspects of clinical recommendations and support plan development.

Community Connector Services (CES Waiver)

Community Connector Services for eligible children on the CES waiver are individualized, person-centered supports and services provided in various locations supporting integration and inclusion in their community. Settings of service delivery may include the home, community centers, parks, or other public places.

Mentorship Services (SLS Waiver)

Mentorship services for those eligible on the SLS waiver are tailored to meet the goals outlined in the individual's person-centered service plan. Mentorship services occur is settings relevant to the needs and preferences of the individual such as their home, in a community setting, or day program. Services are most successful when delivered in a location that is flexible and adaptable.

We provide mentorship services that involve community integration, promote independence, self-determination and self-advocacy.  These services can empower individuals with disabilities to be active and engaged members of their communities, especially when community-based activities are tailored with the individual in order to meet their own interests and goals

Mentors use various teaching strategies when working on program goals such as sharing personal experiences, and role-playing scenarios as well as connecting the individual with additional resources in their community.

SLS Parent education and training in child and infant care available for eligible parents receiving these waiver services.  Assistance with coordinating SLS services with other specialized services in the community may be provided to parents.

Parent Education Services (CES Waiver)

Parent Education services assists expectant parents,  current parents, those pursuing reunification with their child, or other relevant caregivers or stakeholders (such as individuals who provide unpaid support, training, companionship or supervision to parents) in developing and supporting appropriate, parenting skills to meet the needs of the individual child. 

Training is individualized and focused on the health, welfare and developmental needs of the child, building and reinforcing necessary parenting skills and coordinating with informal and other formal supports.
