● Request corrections to any inaccurate or incomplete information in the individual's or client's medical record.

● Request confidential communication of health information through specific means or to an alternate address.

● Ask FBEC to limit the use and disclosure of the individual's or client's health information.

● Receive a list of disclosures of the individual's or client's health information made by FBEC.

● Obtain a copy of this privacy notice.

● Designate a representative to act on your behalf regarding the individual's or client's health information.

● File a complaint if you believe FBEC has violated the individual's privacy rights.

Your Choices

You have the following choices regarding the use and sharing of health information:

● Disclose information to family, friends, or others involved in an individual's or client's care.

● Opt-in or opt-out of the use of information for marketing purposes or the sale of health information. FBEC does not use information for marketing purposes of any kind or engage in practices related to the sale of health information.

FBEC may use and disclose health information for the following purposes:

● Providing treatment to an individual or client.

● Conducting necessary administrative functions within FBEC and communicating with an individual's healthcare policy.

● Billing for services provided to the individual or client.

● Assisting in public health and safety matters.

● Engaging in research activities (with appropriate authorization).

● Complying with legal obligations.

● Collaborating with medical examiners, funeral directors, and other authorized entities.

● Addressing workers' compensation, law enforcement, and government requests.

● Responding to lawsuits and legal actions.

FBEC is committed to maintaining the privacy and security of those seeking information regarding our services, and an individual's or client's protected health information. We will promptly notify you in the event of a privacy or security breach in accordance with local and federal regulations. FBEC adheres to the duties and privacy practices described in this notice and will provide you with a copy upon enrollment. We will only use or disclose an individual's or client's health information as described, unless authorized in writing by you. You may revoke any authorization in writing at any time.
